Insurance

Services may be covered in full or in part by your health insurance or employee benefit plan. Community Tree Counseling's billing company will submit claims on your behalf, so you will need to present your insurance card to at the initial visit so that the billing company has the necessary information on your policy to submit the claim. You will be responsible for paying any portion of fees that insurance does not cover. Fees are due at the time of service. Please check your coverage carefully prior to your first appointment by asking the following questions:

  • Do I have mental health insurance benefits for in network and out of network providers?
  • Is Community Tree Counseling (Marissa Mitchell, LMFT) and in network or out of network provider?
  • What is my deductible and has it been met?
  • Do I have a co-pay per session?
  • How many sessions per year does my health insurance cover?
  • What is the coverage amount per therapy session?
  • Is approval required from my primary care physician?
  • Do I need any other pre-authorization?

Accepted In-Network Insurance Plans: Aetna, Behavioral Healthcare Providers, Blue Cross Blue Shield, Medica, Medical Assitance, Preferred One, Ucare, United Behavioral Health, and United Healthcare.

Accepted Out-of-Network Insurance Plans: Cigna and Health Partners.

Reduced Fee
Reduced fee services are available on a limited basis.

Payment
Accepted forms of payment include cash, check, and credit card.

Cancellation Policy
If you do not show up for your scheduled therapy appointment or you have not notified Community Tree Counseling at least 24 hours in advance, you will be required to pay the full cost of the session. Please be aware that insurance companies do not pay for missed appointments, so you will be responsible for the full amount.
